Disconnected from real‑world delivery
Many tools are built for reporting rather than fieldwork. The result is added burden, low adoption, and incomplete data that does not reflect real implementation.
Huddy Health partners with organizations to build scalable, AI- and software-enabled tools that bring quality, trust, and impact to healthcare delivery.
Evidence-based interventions improve health outcomes, yet many do not effectively scale. Without digital tools designed to support workflows, care coordination, training fidelity, and outcomes measurement across settings, teams, and communities, preventable poor health outcomes slip through the gaps. This is exacerbated for communities with the greatest need.
Many tools are built for reporting rather than fieldwork. The result is added burden, low adoption, and incomplete data that does not reflect real implementation.
Sustained nurse-family relationships drive quality care. Without structural support, people are left to navigate fragmented systems alone.
Programs must flex across languages, cultures, staffing models, and service environments without drifting from what makes them effective.
Effective interventions rely on people, training, and local context. Without repeatable workflows, the program becomes difficult to sustain and expensive to expand.
We turn proven programs into practical tools and workflows that teams can use, measure, and sustain. This helps organizations deliver services consistently, track results, and better respond to families’ needs.
Solutions for the real world Effective care centers on individuals and their families, with nurses, other clinicians, and community partners coordinating to address clinical and social needs together. We build digital solutions grounded in science and equity, designed for this implementation reality.
From protocol to workflow Our digital tools convert protocols into structured steps that seamlessly match field reality, including outreach, engagement, intervention delivery, follow-up, referrals, and handoffs.
Fidelity with flexibility We ensure that core components are met, while supporting local adaptation across teams, culture, language, and operational constraints.
Measurement without added burden Our digital tools capture implementation and outcomes data as part of everyday work, allowing teams to learn and improve without adding extra burden.
Safe Families is The Center for Latino Adolescent and Family Health (CLAFH)'s community-centered initiative, supporting substance use prevention and recovery through nurse-guided engagement, family support, and coordinated services. Huddy Health partnered with CLAFH to create software that proactively reaches families, assesses needs, and connects them to appropriate care.
Millions of people are at risk of or living with substance use disorder, and hundreds of thousands of lives are lost due to overdose each year. Communities have limited access to prevention, treatment, and recovery. They are faced with barriers to naloxone/Narcan access, and workforce strain makes consistent outreach, follow-up, and coordination hard to sustain.
REIS is a workflow toolkit for intervention delivery that supports outreach, recruitment, structured evaluations, intervention delivery, follow-up, coordination, and reporting. It is built to help teams run evidence-based programs with fidelity in real-world settings.
"Huddy Health transformed how we respond to the overdose crisis in our community. Through the REIS digital platform, we were able to reach more families, deliver our evidence-based programs consistently, and place life-saving naloxone directly into the hands of those who needed it most. What once felt constrained by existing technology and capacity, now feels scalable and sustainable."
We build tools that work in the real world
Our evidence-based solutions reflect the lived experience of individuals and families. They are designed to integrate clinical and social care, reaching underserved populations in culturally responsive, interprofessional, and sustainable ways.
Systems Thinking & Program Design We map stakeholders, care workflows, constraints, incentives, and handoffs to design programs that can be implemented and evaluated in real-world settings.
Practical AI We believe AI is a tool, not a strategy. We apply it where it meaningfully reduces friction and improves follow-through, with privacy and safety protections appropriate for healthcare realities.
Configurable Software We build flexible software that supports the day-to-day work of intervention teams, while helping organizations track family engagement and respond in real-time.
Implementation & Adoption Even strong models fail when adoption fails. We support rollout through training, playbooks, and feedback loops that help teams integrate new tools into their daily work.
Measurement, Evaluation & Research Our tools are designed to support research and continuous program learning, making evaluation practical instead of painful.
We partner with organizations committed to scaling evidence-based solutions to complex health challenges. We look for partners who believe great care is both clinically and socially grounded, built on teamwork, trust, and follow-through.
Turn IRB-approved models into operational workflows with participant tracking, structured assessments, and dashboards for reporting that can be deployed, measured, and scaled.
Coordinate services, track follow-through, and reduce gaps across complex systems with clear service blueprints, workflow maps, and implementation plans supported by practical training and SOPs.
Track what happens after discharge across community partners and over time, using workflow automation and thoughtful data capture design that minimizes burden on clinical staff.
Access implementation-level data on community programs through pilot support, iteration cycles, and evaluation reporting that produce export-ready datasets to demonstrate value to stakeholders.
We tailor our approach to your program model, constraints, and success metrics, from early alignment through field testing and scale.
Our Purpose & Mission: To help families actively participate in their care, supported by nurses and community systems. To expand family-oriented care for all stakeholders through systems thinking, healthcare expertise, and AI- and software-enabled healthcare workflows.
Our Vision: A community healthcare ecosystem where families are engaged partners, nurses are empowered interventionists, and care teams have tools that sustain prevention, treatment, and follow-up.
Care is collaborative Lasting outcomes come from working across families, clinicians, and community partners as equals.
Trust is foundational We build trust from the ground up by respecting lived experience and following through on our shared goals.
Equity is non-negotiable We design and act with responsibility to families, interventionists, and communities, not just systems or metrics.
Technology should expand human connection We build systems and tools that strengthen relationships between families, care teams, and communities.
Innovation must lead to measurable impact We hold our work to a high standard, crafting solutions that are accountable to outcomes, not just ideas.
Honest communication We keep families and care teams informed, so they always know what to expect.
Huddy Health is a boutique firm by design. We partner deeply, not broadly. Working at the intersection of care delivery, intervention research, and software engineering, we help move evidence-based interventions from concept to operational reality.
Our work is built alongside teams in community health, intervention research, and real-world implementation.
Whether you are building a new intervention or strengthening an existing program, Huddy Health helps you operationalize delivery, measurement, and follow-through at scale. If you believe families and nurses are essential to sustainable impact, let's talk.
