We are working on a software approach that bridges the gap between healthcare and social services.
In today’s current landscape, care is focused on an individual patient with a clinician reacting to their singular potential health concern. We started our research by looking at social determinants of health (SDoH) and the benefits of zooming out to the larger picture of an individual’s life circumstances to better understand their needs. In the report, Ending Unequal Treatment – Strategies to Achieve Equitable Health Care and Optimal Health for All, the researchers found that creating an introprofessional health system between clinicians and social workers would achieve positive clinical outcomes. That objective brought us to a new idea: dynamic changes in health software frameworks that include families and communities to support the report’s reimagining of healthcare.
We strive to implement an evidence-based program that collects the right data, at the right time, to the right people who can use and analyze it effectively.
