The ADHD Initial Assessment is an 18-item questionnaire that measures indicators of ADHD. The assessment asks you to recall the frequency of your symptoms over 30-days.

You and your doctor can use this Compass to determine your risk for ADHD. It features 5-point Likert scale responses corresponding to the frequency of symptoms (0 = never/rarely, 1 = sometimes, 3 = often, and 5= always). Your scores range from 0 to 90, with lower scores indicating lower risks. The assessment has a cut-off score of 49. You should consult with a medical professional to determine your next steps.
